Conduct outreach and home/community assessments to evaluate needs and social determinants of health, educate members on preventive and chronic care, and coordinate with providers and internal teams. Track and report outreach effectiveness and quality metrics (e.g., HEDIS, CAHPS) while documenting interactions and maintaining compliance with state and federal regulations.
