  • •Conduct member outreach via phone or home visits, discuss care/service plan next steps and resources, and provide ongoing education as appropriate.
  • •Coordinate care activities based on the care/service plan and work with healthcare and community providers, partners, and members/caregivers to accommodate changes or progress.
  • •Handle member and provider inquiries related to care plans/services and facilitate services through coordination with care managers and practitioners.
  • •Document and maintain member records per state and regulatory requirements, including distributions to providers as needed, and support assessments/screenings when required.

Key Requirements

  • •High school diploma or GED, plus 1–2 years of related experience.
  • •For the Arkansas Total Care plan: a bachelor’s degree in a social science/health-related field, or a high school diploma with at least one year coordinating care for developmentally/intellectually disabled or behavioral health clients.
  • •Must reside in Arkansas or a border city.
  • •Must be safety sensitive in Arkansas, including a valid driver’s license, child and adult maltreatment check (before hire and recurring), and drug screen (at time of hire and recurring).
  • •Support members throughout Lincoln, Cleveland, Drew, and Bradley counties in Arkansas.
