Nurse Practitioner (Remote, SC License Required)

Provide phone and video-based post-hospital discharge follow-up care for patients in a Transitions of Care program. Conduct telephonic outreach and scheduled visits focused on medication reconciliation, symptom screening, and ordering appropriate follow-up labs, medications, or supplies. Build trust with patients, explain Galileo’s care model, coordinate across hospital/ED, pharmacies, and community providers, and collaborate with the nursing team to support population health goals.
