Key Responsibilities

  • •Review your case load, prepare technology and tools, and navigate your schedule to service patients.
  • •Conduct virtual adult and/or pediatric wellness visits, health assessments, and related services.
  • •Perform health risk assessments, medication reviews, and cognitive screenings, providing patient education.
  • •Coordinate patient care by arranging blood draws and evaluating vital signs in collaboration with in-home Sprinters.
  • •Manage care coordination including preventive interventions, medication management, referrals, community resources, and documentation.
