Virtual Family Practice Nurse Practitioner - Remote (North Carolina, South Carolina Licensed)

Provide virtual adult and/or pediatric wellness visits to patients in their homes, conducting health risk assessments, medication reviews, cognitive screenings, and patient education. Manage proactive, patient-centric care while coordinating with Sprinters for vital signs and in-home support such as blood draws. Handle documentation, referrals to specialists, and community resource coordination, using charting tools and HIPAA-secure messaging to maintain high-quality, autonomous care.
