Senior Post Acute Transition Specialist - Enterprise

Serve as an executive-level account manager and the principal post-acute authority for a flagship Enterprise Practice. You’ll build partnerships with practice leadership and hospital-based care teams, independently lead post-acute initiatives, and align stakeholders through recurring forums. Using claims data, clinical insights, and performance analytics, you’ll develop operational workflows that improve patient wellness metrics (length of stay and readmissions) to strengthen ACO financial performance.
