•Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.

•New graduates who are board-eligible and actively pursuing licensing are encouraged to apply.
•Provide high-quality, patient-centered primary care, diagnosing and treating acute and chronic conditions.

•Wamiliarity with quality initiatives such as Chronic Care Management, Transitional Care, Remote Patient Monitoring.
