Job Summary

Coordinate whole-person care for individuals with serious mental illness, substance use disorder, homelessness, and other high medical needs. Maintain an evidence-based caseload using relationship-driven engagement, telephonic and SMS outreach, and development of Health Action Plans/Shared Care Plans. Meet monthly encounter metrics and help remove barriers by scheduling appointments, managing referrals, and coordinating visits with community and care team partners. Work primarily from home with 2–3 community on-site visits per month.

•Use relationship-based strategies to support members with social support navigation and trust-building.
•Conduct periodic telephonic and SMS outreach to ensure timely follow-up.
•Work with members to identify health and wellness goals and incorporate them into Health Action Plans/Shared Care Plans.
•Coordinate connection to community supports by scheduling appointments, managing referrals, and ensuring timely follow-ups; collaborate on care issues through systematic case reviews.

•Strong empathy and cultural fluency, with the ability to collaborate in a multidisciplinary team.
•Strong technical comfort with technology, including CRM databases, basic Excel/Word, email, and video conferencing; valid driver’s license and auto liability insurance.
