Key Responsibilities

•Serve as the attending physician for patients in each assigned SNF.
•Collaborate with NPs/PAs to manage daily patient care.
•Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
•Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
•Engage in ongoing professional development and participate in relicensing requirements as needed.

Key Requirements

•Active U.S. medical license (MD/DO) and board-certified or board-eligible in an appropriate specialty.
•New graduates who are board-eligible and actively pursuing licensing are encouraged to apply.
•Qualified specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
