Attending physician responsible for primary care and ongoing management of patients in assigned skilled nursing facilities (SNFs). Provides high-quality, patient-centered care, collaborates with NPs/PAs, and participates in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
