Key Responsibilities

  • •Develop, assess, and coordinate holistic care management activities to support quality, cost-effective healthcare outcomes.
  • •Create and maintain personalized service care plans, identify needed providers/specialists/community resources, and recommend best-outcome plans.
  • •Coordinate between members (and/or families/caregivers) and the care provider team to ensure services are accessible.
  • •Provide resource support and education to members and families/caregivers on procedures, provider instructions, service options, referrals, and benefits.

Key Requirements

  • •Bachelor’s degree and 2–4 years of related experience, or an equivalent combination of education and qualifying case management experience.
  • •Experience interviewing and assessing member needs and knowledge of caseload management and casework practices.
  • •Knowledge of determining eligibility for DHSS programs and applicable Federal/State law as it applies to those programs.
  • •Ability to solve problems and locate community resources, and to collaborate with caregivers, state agency representatives, and providers.
  • •Must reside in Miami-Dade County, FL.
