Key Responsibilities

  • •Coordinate care activities based on member care/service plans and work with healthcare and community providers and partners.
  • •Conduct member outreach telephonically or through home visits to discuss care plans, next steps, resources, questions, and concerns.
  • •Serve as a liaison alongside care managers and providers to facilitate member services and continuity of care.
  • •Perform service assessments/screening for members with some complex needs and document member care needs per state and regulatory requirements.
  • •Triaging, adjusting, and escalating complex member/provider requests and supporting education on benefits and available resources.
