Key Responsibilities

  • •Serve as the primary point of contact for members with complex medical, behavioral, and social needs.
  • •Complete new patient onboardings and provide care support through in-person visits at homes, community sites, or partner organizations.
  • •Assess member needs during visits, provide ongoing support, and encourage adherence to care plans.
  • •Coordinate services, appointments, and referrals across primary care, behavioral health, and social support resources.
  • •Maintain accurate HIPAA-compliant documentation and collaborate with internal care team members in care meetings to align on care plans.
