Key Responsibilities

  • •Coordinate care plans for multiple patients to optimise patient experience.
  • •Collaborate with patients, healthcare providers, care teams, facilities, and schemes to ensure eligible patients receive coordinated care throughout the journey.
  • •Provide tele-health support by reviewing virtual patient wellbeing and ensuring monitoring stays within required clinical parameters.
  • •Monitor and review patients’ biometrics and immediate wellbeing with continuous record keeping on multiple electronic platforms.
  • •Support reporting, analysis, and feedback to improve care coordination outcomes.

Key Requirements

  • •Degree/Diploma leading to qualification as a Professional or General Nurse, plus a Diploma/Degree in a relevant healthcare field.
  • •Qualified Paramedic / Emergency Care Practitioner / Professional Nurse with at least 3 years’ prior experience working with patients in an administrative, clinical, or care coordination role.
  • •Ability to provide tele-health support and review virtual patient wellbeing while ensuring monitoring remains within required clinical parameters.
  • •Strong organisational skills, attention to detail, and knowledge of relevant policies and procedures.
  • •Computer literacy (MS Office) with excellent English communication (written and verbal), feedback, and follow-up skills.
