Key Responsibilities

  • •Conduct phone-based assessments and use peer coaching strategies (health education, friendly check-ins, and resource linking) to address health and social service needs.
  • •Follow provided processes and policies, and be prompt with scheduled engagements, follow-through, documentation, and progress tracking.
  • •Maintain appropriate conversation topics, build rapport, and provide a positive, welcoming experience for community members.
  • •Use community resources, social services, and partnerships to meet needs and communicate questions/concerns to staff.
  • •Participate in orientation and ongoing training and support community outreach and evaluation as needed.

Key Requirements

  • •Must be located in Santa Clara County.
  • •Hold a valid CHW certificate pathway curriculum completion in required skill/training areas, or meet the work experience pathway (at least 2,000 hours as a CHW in paid or volunteer roles within the prior three years).
  • •Reliable transportation and a flexible schedule for face-to-face outreach.
  • •Strong written and verbal communication skills for engaging by phone, video, and in person with staff and community members.
  • •Ability to use web-based systems effectively and maintain confidentiality in line with HIPPA guidelines.
