Key Responsibilities

  • •Identify, outreach, engage, and enroll eligible individuals into the program virtually (phone, text, email) and in person at clinics and community partner locations.
  • •Listen openly to members, meet them where they are, and handle engagement with trust-building communication and persistence through multiple outreach attempts.
  • •Provide education to partners and explain Enhanced Care Management services and benefits to eligible members.
  • •Leverage community knowledge and best-in-class customer service skills to gain trust and enroll individuals into the program.
  • •Manage the end-to-end enrollment funnel, provide consistent updates on funnel stages, and meet enrollment metrics.
