Bilingual Case Management Specialist (Remote Flexible, Spanish Speaking)

Provide whole-person, interdisciplinary care support by managing a caseload and engaging members through relationship-based outreach and evidence-based approaches. Conduct telephonic and SMS follow-up, help create and maintain Health Action/Shared Care Plans, and coordinate community and care-management appointments. Collaborate in systematic case reviews with nurses, behavioral health staff, and other care team members, meeting encounter metrics while ensuring continuity of program participation.
