Key Responsibilities

  • •Assist in developing, assessing, and coordinating holistic care management activities for quality, cost-effective healthcare outcomes.
  • •Develop ongoing long-term care service care plans and identify providers, specialists, and community resources needed for members.
  • •Coordinate between members/families/caregivers and the care provider team to ensure services are accessible.
  • •Monitor care plans and member status/outcomes, and recommend updates based on identified needs.
  • •Collect, document, and maintain member information and care management activities to ensure compliance; may conduct home/site visits and provide member education.

Key Requirements

  • •Requires a Bachelor's degree and 2–4 years of related experience.
  • •Bachelor’s degree in health, human, social work, or education services with qualifying experience; or high school plus qualifying case management experience for aged or specified populations.
  • •Provide care management by evaluating member needs, resources, and recommending and/or facilitating service care plans for long-term care members.
  • •Communicate effectively with long-term care members and their families/caregivers, as well as care provider teams.
  • •May need to conduct home or other site visits to assess member needs and collaborate with providers and partners.
