Job Summary

Optimize patient outcomes through telephonic medication management interventions. Counsel patients on medication adherence, polypharmacy, and high-risk medication use, using evidence-based recommendations and focused clinical chart reviews. Collaborate with physicians and care teams to improve medication management workflows, identify gaps, and support “test and learn” pilot programs. Escalate complex cases appropriately and maintain quality and performance goals tied to STARs/HEDIS.

Key Responsibilities

•Counsel patients via telephonic outreach on medication adherence and related issues (including adherence barriers, polypharmacy, and high-risk medication use) and provide evidence-based recommendations to patients and Practice Care Teams.
•Perform focused clinical chart reviews to inform patient interventions.
•Obtain and document patient commitment to medication adherence and address barriers in real time.
•Collaborate with physicians and clinical/administrative staff to optimize the medication management workflow and improve patient experience.
•Escalate complex clinical cases to prescribers or interdisciplinary teams as appropriate and meet performance goals (e.g., STARs, HEDIS).

Salary: USD 115,000 - 135,000 annually

Key Requirements

•Doctor of Pharmacy (PharmD).
•Active, unrestricted pharmacist license in the state of practice, with valid multi-state licensure in at least two listed states; willingness to obtain additional licenses within 6 months of hire.
•Minimum 3 years of experience in value-based care, ambulatory care, or community pharmacy settings.
•In-depth knowledge of pharmacotherapy and pharmacy practice.
•Strong verbal and written communication skills; proficiency with telephony platforms and Electronic Health Record (EHR) systems.
