Key Responsibilities

  • •Serve as the primary point of contact for assigned members, building trust and offering personalized support.
  • •Help members understand and use the Twin platform to drive informed lifestyle changes.
  • •Deliver structured conversations at a defined frequency that add value, instill motivation, and drive positive outcomes.
  • •Provide individualized nutrition and lifestyle coaching focused on remissioning chronic metabolic diseases and improving overall health.
  • •Interpret and review member health data daily and proactively manage adherence, retention, and reactivation, including handling escalations and collaborating with internal teams.

  • •Ability to act as a primary point of contact for assigned members and deliver a consistently high-touch experience.
  • •Ability to bridge health and technology by helping members understand and leverage the Twin platform.
