Key Responsibilities

•Improve the quality and completeness of clinical documentation in the legal medical record under clinical documentation guidelines and established policies.
•Coordinate with physicians, HIM, and coding staff to facilitate appropriate documentation supporting reimbursement outcomes for each level of service.
•Educate physicians on clinical documentation needs, guideline changes, and coding/reimbursement opportunities on an ongoing basis.
•Evaluate medical records using medical terminology and procedures to identify documentation and reimbursement opportunities.
•Perform concurrent acute care (inpatient) medical record monitoring of diagnoses, treatments, and follow-up entries; use queries to obtain missing information when needed.

Key Requirements

•Have at least one of: Registered Nurse license (any state) or RHIA, RHIT, or CCS credential.
•1+ year of Acute Care (inpatient) Concurrent Clinical Documentation Specialist experience.
•CCD S (Certified Clinical Documentation Specialist - ACDIS) or CDIP (Certified Documentation Practitioner - AHIMA) credential required.
•Experience with concurrent inpatient facility coding/clinical documentation improvement and medical record review to validate documentation accuracy.
•Ability to obtain missing clinical documentation information via query to support appropriate coding and billing.
