Key Responsibilities

  • •Deliver virtual primary care to Medicare patients with chronic, complex, and overlapping medical, behavioral health, and social needs.
  • •Collaborate with physicians, nurse practitioners, and care managers to provide high-quality clinical care and support care coordination.
  • •Supervise nurse practitioners providing multidisciplinary care for patients with serious mental illness/substance use disorder and/or experiencing homelessness/high medical needs.
  • •Enable best-in-class virtual health care by partnering with NPs using internal care delivery technology and strong clinical judgment.
  • •Help develop and refine clinical programs and contribute to the launch of Medicare across multiple states.

Key Requirements

  • •2+ years of clinical experience serving patients with complex social and healthcare needs, with passion for equitable care.
  • •Board certification or eligibility as a Family Physician; eligible to enroll in Medicare.
  • •Technology-savvy with digital-first healthcare experience (e.g., telemedicine) and comfort working across tech platforms.
  • •Physically located in California with a quiet, HIPAA-compliant, internet-connected space.
  • •Active CA medical license with active CA DEA (or willingness to obtain); comfortable working across the lifespan and experience with Medicaid/FQHC and Medicare chronic disease patients.
