Job Summary

The Intake Coordinator supports the Refugee Health Promotion Program by evaluating client needs through health and wellness assessments, coordinating enrollment, and ensuring all documents are accurately processed. Key Responsibilities

•Familiarize yourself with program guidelines and requirements for grant and funding sources; operate as a cultural liaison between clients and staff, service providers, and volunteers to ensure culturally and linguistically accessible services.
•Lead out in client recruitment, eligibility screening, and enrollment.
•Work with clients to identify strengths and barriers to health and wellness integration, then establish a plan of care.
•Communicate client needs and feedback to teammates; keep accurate records of client documents and maintain up-to-date case files.
•Assist in drafting reports (narrative, data, financial) to grantors, donors, and volunteers.
