Job Summary

Support a whole-person, interdisciplinary care model by managing a caseload and using relationship-based, evidence-based approaches to help members engage and achieve health goals. Perform periodic telephonic and SMS outreach, coordinate Health Action Plan/Shared Care Plan goals, and connect individuals to community supports through referrals and follow-ups. Collaborate with the Enhanced Care Management team through case reviews and delegated task support, while meeting encounter metrics and removing barriers to program continuation.

Key Responsibilities

•Maintain an ongoing caseload using evidence-based approaches to promote engagement and health goal achievement.
•Use relationship-based strategies to support members with social support navigation, especially those hesitant or distrusting of the healthcare system.
•Conduct periodic telephonic and SMS outreach to ensure timely follow-up.
•Work with members to identify health/wellness goals and incorporate them into Health Action Plan/Shared Care Plan.
•Collaborate with the Enhanced Care Management team through case reviews and support members by scheduling appointments, managing referrals, and ensuring timely follow-ups.

Travel: Low travel

Salary: USD 22 - 25 hourly

Key Requirements

•1+ years of general work experience (case management preferred).
•Must be physically located in California.
•Bilingual: English/Spanish.

•Strong technical skills and comfort with technology, including experience with CRM databases and basic Excel, Word, email, and video conferencing.
