Key Responsibilities

  • •Provide high-quality, patient-facing primary care in members’ homes across New Jersey.
  • •Manage a panel of medically complex Medicare Advantage members and take accountability for clinical outcomes.
  • •Conduct comprehensive in-home assessments of medical, functional, behavioral, and social needs.
  • •Lead post-discharge and transitional care visits to support safe recovery and reduce avoidable readmissions.
  • •Identify clinical status changes early, manage chronic conditions and acute needs, and collaborate with the interdisciplinary care team.

Key Requirements

  • •MD or DO degree.
  • •Board certification in Internal Medicine, Family Medicine, Emergency Medicine, or a related primary care field.

  • •Comfort caring for medically complex, frail, and socially vulnerable patients.
