Home Based Primary Care Physician (MD/DO)

Provide relationship-centered, outcomes-driven primary care for Medicare Advantage members with complex medical, functional, and social needs in their homes across New Jersey. Manage a defined panel, conduct comprehensive in-home assessments, lead post-discharge and transitional care visits, identify clinical changes early, and coordinate next steps with an interdisciplinary team. Use Clover Assistant and other clinical technology to guide decisions, document thoughtfully, and close care gaps to reduce avoidable readmissions.
