Bilingual Case Management Specialist (Remote Flexible, Spanish Speaking)

Coordinate whole-person case management for individuals with serious mental illness, substance use disorder, and/or homelessness and high medical needs. Maintain an ongoing caseload using evidence-based, relationship-driven engagement; conduct telephonic and SMS outreach; and support health/wellness goal setting via health action/shared care plans. Work with interdisciplinary care teams through case reviews and delegated tasks, while meeting encounter metrics and connecting members to community supports through scheduling, referrals, and follow-ups.
