Key Responsibilities

  • •Lead implementation of complex regional initiatives, including grant-funded programs, provider network and workforce programs, telehealth initiatives, and healthcare-community partnerships.
  • •Translate proposals, contracts, grants, and priorities into actionable project plans with milestones, owners, timelines, dependencies, and performance measures.
  • •Manage multiple programs and workstreams simultaneously; track progress, deliverables, risks, budgets where applicable, and key performance indicators.
  • •Coordinate cross-functionally as the central point of coordination across workforce development, community outreach, provider network, operations, product, clinical, impact, sales, and payer teams.
  • •Build and maintain partner relationships and support onboarding, readiness assessments, meetings/working groups, and ongoing technical assistance to sustain implementation and outcomes.

Key Requirements

  • •4–7 years of experience in healthcare, public health, community health, healthcare operations, Medicaid, population health, program management, or a related field.
  • •Experience coordinating programs or projects across multiple organizations and stakeholder groups.
  • •Strong project and program management skills for planning, milestone tracking, documentation, and stakeholder coordination.
  • •Ability to independently manage projects and move work forward with minimal highly structured processes.
  • •Strong communication, facilitation, relationship-building, and problem-solving skills; experience with CBOs/CHWs/Tribal or Medicaid-related stakeholders is preferred.
