•Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.

Key Requirements

•Active U.S. medical license and board-certified or board-eligible in Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, or Geriatric Medicine; new graduates board-eligible encouraged to apply.
•Experience with primary care in post-acute settings and supervising clinical staff (NPs/PAs)
•Ability to diagnose and treat acute/chronic conditions with a patient-centered approach
•Comfort participating in quality initiatives such as Chronic Care Management, Transitional Care, and Remote Patient Monitoring
•Willingness to work in a part-time schedule with flexibility and autonomy
