Key Responsibilities

  • •Independently manage acute and chronic conditions, including remote treatment when suitable and in accordance with TruDoc policies and procedures.
  • •Compile patient histories, formulate diagnoses, order diagnostic tests, and prescribe appropriate treatments.
  • •Provide in-home assessments and remotely monitor patients using telemonitoring software.
  • •Enroll patients in the Chronic Care program and perform complete assessments for those who may benefit.
