Virtual Family Practice Nurse Practitioner - Remote (Oklahoma Licensed)

Provide virtual adult and/or pediatric wellness visits from patients’ homes, performing health assessments, health risk assessments, medication reviews, and cognitive screenings. Coordinate proactive patient care and document findings, partnering with in-home clinical team members to arrange blood draws and address patient needs. Use laptops or mobile devices for charting and HIPAA-secure messaging for care coordination while working independently with a patient-centric, compassionate approach.
