Key Responsibilities

  • •Develop, assess, and coordinate care management activities based on member needs.
  • •Create or contribute to personalized care plan/service plans and identify needed providers, specialists, and community resources.
  • •Provide psychosocial and resource support to members/caregivers to access services such as employment, education, housing, food, and foster care resources.
  • •Coordinate between the member/family/caregivers and care provider team to ensure timely access to care and services.
  • •Collect, document, and maintain member information and care management activities to ensure compliance with applicable regulators.

Key Requirements

  • •Bachelor’s degree with 2–4 years of related experience.
  • •Graduate from an Accredited School of Nursing if holding clinical licensure.
  • •Bachelor’s or Master’s degree in a healthcare-related field (social work, sociology, psychology, public health) and 2–4 years of related experience for the Mississippi Magnolia Health plan.
  • •LSW or LMSW preferred.
  • •Experience working with the Foster Care population.
