Key Responsibilities

  • •Identify, outreach, engage, and enroll eligible individuals into the program virtually and in-person at partner locations.
  • •Listen to eligible members and adapt outreach approach to support successful enrollment.
  • •Educate partners about Pair Team services and link members to the Enhanced Care Management program.
  • •Explain Enhanced Care Management services and benefits, addressing concerns and building trust.
  • •Manage the end-to-end enrollment funnel and provide consistent updates on funnel stages, delays, and blockers.
