Job Summary

Coordinate holistic care management for long-term support services members with physical disability and aging waiver needs. Assess members’ needs and available resources, develop and monitor personalized service care plans, and connect members and families/caregivers with providers and community resources. Document and maintain member information to ensure compliance, provide education on services and benefits, and perform home/onsite visits to ensure care plans meet outcomes.

•Monitor care plans and member status/outcomes and recommend updates based on changing needs.
•Collect, document, and maintain care management information to ensure compliance and provide education on services, procedures, referrals, and benefits.

Key Requirements

•Bachelor’s degree and 1 year of related experience.
•For Illinois: RN licensed in Illinois.
•For Illinois: Bachelor’s or Master’s degree in a human services-related field.
•For Illinois: LPN with 1 year of experience in conducting comprehensive assessments and providing formal service for the elderly.
•Experience with case management, advocacy, or home visits/community travel (preference).
