Key Responsibilities

  • •Assess member needs and resources, and recommend or facilitate the best plan for outcomes.
  • •Develop and support ongoing long-term care service plans and identify required providers, specialists, and community resources.
  • •Coordinate between members/families/caregivers and care provider teams to ensure services are accessible.
  • •Monitor care plans and member status/outcomes and recommend updates based on changing needs.
  • •Collect, document, and maintain care management information to ensure compliance and provide education on services, procedures, referrals, and benefits.

Key Requirements

  • •Bachelor’s degree and 1 year of related experience.

  • •For Illinois: LPN with 1 year of experience in conducting comprehensive assessments and providing formal service for the elderly.
  • •Experience with case management, advocacy, or home visits/community travel (preference).
