Key Responsibilities

  • •Facilitate in-room telemedicine visits and schedule provider appointments (acute, follow-up, routine).
  • •Prepare residents during provider visits, including positioning and documentation; update EMRs with medical histories.
  • •Support care coordination and transitions of care, including referrals, follow-up appointments, and prior authorizations.
  • •Reinforce provider instructions by educating residents on nutrition, fall prevention, medication reminders, and wellness; route concerns to licensed staff/providers.
  • •Maintain accurate documentation in the EMR and support regulatory compliance and quality initiatives, including attending IDT meetings.
Travel: Low travel

Key Requirements

  • •Graduate of an accredited Medical Assistant (MA) program.
  • •Certified Medical Assistant (CMA) preferred.
  • •Prior Medical Assistant experience in a clinical or care-coordination setting; Health Plan/Hospice Liaison experience preferred.
  • •Superior interpersonal skills with professional demeanor.
  • •Experience charting in an EMR and strong detail orientation, problem-solving, and communication skills.
