Key Responsibilities

  • •Develop, assess, and facilitate complex care management activities for members’ physical needs.
  • •Evaluate member needs, barriers to care, and social determinants of health; recommend and/or facilitate the best plan for outcomes.
  • •Develop ongoing care/service plans and collaborate with providers and community resources to address unmet needs.
  • •Coordinate between members/families/caregivers and the care provider team to ensure person-centered care.
  • •Provide ongoing follow-up and monitoring, collect/document care management information, and educate members and caregivers on disease processes and care options.
