Care Manager (RN) - Foster Care

Develop and coordinate complex care management for members, focusing on physical health needs while understanding behavioral health considerations. Assess barriers to care, social determinants of health, and member priorities to create and update personalized care/service plans. Collaborate with providers and caregivers, provide ongoing monitoring, and deliver education and resource support—while maintaining compliant documentation across state, federal, and payer requirements.
