Key Responsibilities

  • •Assess member needs and barriers to care, then recommend and facilitate care plans for quality, cost-effective outcomes.
  • •Develop or contribute to personalized care plans/service plans and identify needed providers, specialists, and community resources.
  • •Provide psychosocial and resource support to members/caregivers and connect them to local resources such as employment, housing, food, and independent living.
  • •Coordinate between members/caregivers and the provider team to ensure services are accessible in a timely manner.
  • •Collect, document, and maintain member information and care management activities for compliance, and monitor progress/change in condition.

Key Requirements

  • •Bachelor’s degree and 2–4 years of related experience.
  • •Graduate from an accredited school of nursing if holding clinical licensure.
  • •Prefer OB experience.
  • •For Mississippi Magnolia Health plan: bachelor’s or master’s degree in a healthcare-related field (social work, sociology, psychology, public health) and 2–4 years of related experience.
  • •LSW/LMSW preferred.
