Bilingual Case Management Specialist (Remote Flexible, Spanish Speaking)

Maintain an ongoing caseload and use evidence-based, relationship-driven engagement to help individuals with serious mental illness, substance use disorder, homelessness, or high medical needs achieve health goals. Conduct telephonic and SMS outreach, support care planning through shared health action plans, and help connect people to community supports. Collaborate in systematic case reviews with the care team, while meeting monthly encounter metrics and ensuring continuity in the program.
