Job Summary

Serve as a Lead Care Manager in a whole-person, interdisciplinary care model by maintaining a caseload and engaging individuals with serious mental illness, substance use disorder, and/or homelessness or high medical needs. Conduct telephonic and SMS outreach, develop and track health action/shared care plans, coordinate referrals and appointments, and collaborate through systematic case reviews to meet encounter metrics and reduce care barriers.

•Maintain an ongoing caseload and use evidence-based approaches to support engagement and achievement of health goals.
•Use relationship-based strategies to support members with social support navigation and trust-building.
•Conduct periodic telephonic and SMS outreach to ensure timely follow-up.
•Work with members to identify health and wellness goals and incorporate them into Health Action Plans/Shared Care Plans.
•Coordinate care by scheduling appointments, managing referrals, collaborating with internal/external providers, and supporting delegated tasks with the care team.
