Coordinate referrals and community supports, meet monthly encounter metrics, and collaborate on systematic case reviews with the enhanced care management team.

Key Responsibilities

•Maintain an ongoing caseload using evidence-based approaches to promote engagement and health goal achievement.
•Use relationship-based strategies to support members with social support navigation and trust-building.
•Conduct periodic telephonic and SMS outreach and ensure timely follow-up.
•Collaborate with members to identify health and wellness goals and incorporate them into Health Action Plans/Shared Care Plans.
•Coordinate referrals and community support connections, schedule appointments, and participate in systematic case reviews with the enhanced care management team.

Salary: USD 22 - 25 hourly

Key Requirements

•1+ years of general work experience (case management preferred).
•Physically located in California.
•Bilingual (English/Spanish) with strong cultural fluency and empathy.

•Comfort with technology (CRM databases, Excel, Word, email, video conferencing) and valid driver’s license with auto liability insurance.
