Job Summary

Lead end-to-end operations for partnership and referral activities across connected care channels, including home clinical visits, labs, PT, vaccinations, and imaging. Manage appointment and data workflows, oversee lab result delivery against KPIs, and establish KPIs/SLAs to track signed-off outcomes. Serve as an escalation point for service failures, interpret medical reports for patients and families, and partner with IT/Digital teams to improve data-driven digitalization of home healthcare.

Key Responsibilities

•Lead end-to-end operations, execution, delivery, service, and outcomes for partnership and referral activities across connected care channels.
•Schedule home visits using data analysis; analyze appointment data for trends and improvements.
•Collaborate with third-party providers on quality checks and oversee lab result delivery within KPIs.
•Establish KPIs and SLAs for outcome measurement; design and implement SIA execution plans.
•Act as escalation point for service failures; interpret and explain medical reports to patients and their families when necessary.

Key Requirements

•Bachelor’s degree in Medicine and Surgery or Healthcare Management (master’s degree preferable).
•Proven experience in home health care management or a similar leadership role.
•Strong understanding of medical guidelines and procedures.
•Ability to analyze and interpret data for informed decision-making.
•Knowledge of healthcare regulations and compliance (up to 6 years clinical experience required).
