Key Responsibilities

  • •Facilitate in-room telemedicine visits and coordinate acute, follow-up, and routine provider appointments
  • •Prepare residents for provider visits and update EHRs with medical histories to support care plans
  • •Support transitions of care across the post-acute continuum, including referrals, follow-up appointments, and prior authorizations
  • •Reinforce provider instructions by educating residents on nutrition, fall prevention, medication reminders, and general wellness
  • •Maintain accurate EMR documentation and provide support during discharge planning, including collaborating with clinical coordinator and attending IDT meetings
Travel: Low travel

Salary: USD 19 - 24

Key Requirements

  • •Graduate of an accredited Medical Assistant (MA) program
  • •Prior experience as a Medical Assistant in a clinical or care-coordination setting
  • •Certified Medical Assistant (CMA) preferred
  • •Managed Care experience preferred
  • •Experience charting in an EMR
