Key Responsibilities

  • •Serve as the primary point of contact for members with complex medical, behavioral, and social needs.
  • •Meet members in person to complete new patient onboardings, deliver care packages, and collect health readings such as blood pressure.

Key Requirements

  • •Strong empathy, cultural fluency, and ability to collaborate in a multidisciplinary care team while maintaining accurate documentation.
