Care Navigator

Develop and coordinate personalized care plans for members by assessing needs, barriers to care, and available resources. Educate members and families/caregivers on services and benefits, connect them with providers and community resources (including foster care and social supports where needed), and coordinate timely access to care. Document and maintain member information to meet regulatory requirements, monitor progress and status changes, and perform on-site visits as appropriate.
