Care Manager (RN)

Develops and facilitates complex care management for members’ primarily physical care needs, creating personalized care plans and providing education to members and their families. You will evaluate barriers to care and social determinants, coordinate with providers and community resources, and monitor progress and condition changes to update service plans. The role also includes telephonic/digital/home outreach as needed and documentation to ensure compliance with state, federal, and payer regulations.
