Key Responsibilities

  • •Conduct initial intake, collect demographic data, and assess program eligibility.
  • •Develop and maintain individualized service plans with participant-identified goals and benchmarks.
  • •Coordinate referrals and assist with appointment access to connect participants to community resources and services.
  • •Conduct regular participant check-ins via phone, virtual meetings, home visits, or in-office appointments and support case closures when services are complete.
  • •Maintain accurate case files and documentation in a secure database; complete reports and participate in monitoring, audits, and quality assurance activities.

Salary: USD 27.7 - 28.6 hourly
