Provider Liaison - Medical Assistant (Temporary)

Coordinate in-room telemedicine and provider visits for residents in skilled nursing facilities, updating EHRs and supporting documentation. Manage transitions of care across the post-acute continuum by facilitating referrals, prior authorizations, and follow-up appointments, including Home Health and DME order support. Reinforce patient education, distribute provider-approved materials, route concerns to licensed staff, and help ensure discharge and treatment plans are understood and executed.
