Job Summary

Provide preventive and patient-centered care to enrolled Medicare Advantage members through remote visits, including telehealth via Clover’s platform and approved video apps. Conduct comprehensive health assessments, complete documentation and encounter summaries in Clover Assistant/EMR and AthenaNet, order labs, coordinate care transitions after hospitalization, and create personalized care plans with primary care providers. Support quality-measure performance and referrals to network and internal care teams.

Key Responsibilities

•Deliver preventive care to Clover members remotely via Clover’s platform and approved third-party video apps.
•Conduct comprehensive reviews and health assessments, including medical history, physical health, environment, and psycho-social status.
•Document progress notes and prepare member workups and encounter summaries using Clover Assistant and EMR, completing AthenaNet visit documentation and encounter closure per policy.
•Help at-risk members transition from hospital/post-acute settings to home and create personalized care plans in coordination with primary care physicians.
•Close gaps in care by ordering labs, writing prescriptions with collaboration, coordinating physician follow-up, and performing point-of-care labs/diagnostic testing as applicable.

Pay and Benefits

Salary: USD 125

Key Requirements

•Be certified as an Advanced Nurse Practitioner in specialty areas (required).
•Hold a current unencumbered NJ license as an RN or a valid New Jersey medical doctor license.
•Have an active DEA and CDS registration.
•Have at least 1 year of clinical work experience providing direct patient care.
•Graduate with a BSN from an accredited school of nursing and have a current BLS provider card.
