Job Summary

Provide strengths-based, culturally responsive, trauma-informed case management to refugees and immigrants to support long-term self-sufficiency and community integration. Conduct intakes, assess eligibility, create individualized service plans, coordinate referrals, and offer orientation and coaching on U.S. systems and resources. Maintain accurate case documentation in secure databases, track outcomes for grants, and collaborate with internal teams, volunteers, and community partners.

Key Responsibilities

•Provide ongoing case management, including addressing barriers to self-sufficiency and integration across housing, employment, healthcare, mental health, and education.
•Conduct initial intakes and collect demographic data to assess program eligibility.
•Develop individualized service plans based on participant goals and benchmarks, and provide orientation and coaching on community resources and U.S. systems.
•Coordinate referrals and follow up to ensure linkage to services; assist with appointment coordination as needed.
•Maintain accurate case files and participant records in secure databases, track outcomes and metrics, and complete required reporting.

Pay and Benefits

Salary: USD 24.32 - 25.1 hourly

•Ability to serve immigrants and refugees with prior cross-cultural experience required.
•Ability to read and write proficiently in English.
•Flexibility for occasional evening or weekend hours; intermediate computer knowledge including MS Office; valid driver’s license and access to a reliable vehicle.
