Key Responsibilities

  • •Review your case load each day and prepare technology and tools needed to service patients.
  • •Navigate your schedule and partner with in-home team members to support patient care.
  • •Conduct proactive virtual wellness care, including health risk assessments, medication reviews, and cognitive screenings.
  • •Work directly with Sprinters to evaluate vital signs and arrange blood draws.
  • •Provide comprehensive care coordination, including preventive interventions, medication management, referrals, community resources, and documentation.

  • •Active Family Nurse Practitioner license in KY, MI, OH, or IL.
